Patient Forms
To help your appointment start as quickly as possible, you can download and fill out our patient forms ahead of time by clicking on the link below. When the day of your visit arrives, just bring the forms with you, or email your forms to lparksidedental@gmail.com and we’ll get you in to see Dr. Moneyhun right away! This can easily save you 15-20 minutes! Please let us know if you have any issues accessing our forms.
Dental Insurance
We welcome and accept all PPO dental insurance plans, and our team will make the process of using your benefits nice and simple. We’ll go over your plan so you know exactly what is and isn’t covered, and we’ll even file the claim for you to save you from all that paperwork. Financing Options
Do you currently NOT have dental insurance? Or, do you or a loved one require quite a bit of care that won’t be fully covered by your policy? If so, you have the option to break up the cost of your dental work into smaller monthly payments with CareCredit. They are a third-party financier we have worked with for years, and they have already helped many of our patients fit their dental care easily into their budgets.
